About

Direct Relief is a humanitarian aid organization headquartered in Santa Barbara, California, active in all 50 U.S. states and more than 90 countries. Its mission is to improve the health and lives of people affected by poverty or emergencies — without regard to politics, religion, or ability to pay. Nongovernmental, nonsectarian, and not-for-profit, Direct Relief relies entirely on private contributions.

Mission

Direct Relief works to equip health professionals in resource-poor communities to meet the challenges of diagnosing and caring for people in need. It provides appropriate and specifically requested medical resources to community-based institutions and organizations throughout the world and across the United States.

What they do

  • Disaster relief: prepares vulnerable communities worldwide for more frequent, more destructive emergencies, and responds fast when disasters strike — hurricanes, wildfires, earthquakes, and outbreaks — to get medical resources where they are needed.
  • Charitable medicine: operates the largest charitable medicine program in the United States.
  • Community health: supports community-based facilities providing essential health services worldwide.
  • Maternal health: places particular emphasis on protecting women through the critical periods of pregnancy and childbirth.
  • Core functions include local partnerships, needs assessment, resource mobilization, and global logistics.

History

The organization traces its roots to 1945, when Estonian immigrant William Zimdin began sending thousands of relief parcels containing food, clothing, and medicines to people rebuilding their lives in Europe after World War II. He established the William Zimdin Foundation on August 23, 1948, as a California-based nonprofit corporation. The organization was renamed the Direct Relief Foundation in 1957 and Direct Relief International in 1982. In 1962 it became licensed as a wholesale pharmacy to secure prescription medicines for use abroad, building the strict handling protocols and pharmaceutical-industry partnerships that underpin its work today.

Scale and impact

In addition to its California headquarters, Direct Relief maintains registered affiliates in Europe (Direct Relief gGmbH, Frankfurt) and Mexico (Direct Relief Mexico), and a program office in Puerto Rico. Its work has earned a 100% fundraising efficiency rating from Forbes, a place on Charity Navigator's list of the "10 Best Charities Everyone's Heard Of," the #1 spot on CNBC's "Top 10 Charities Changing the World," and inclusion in Fast Company's list of the world's most innovative nonprofits.

Financials

Per its IRS Form 990 for the fiscal year ending June 2025 (via ProPublica Nonprofit Explorer), Direct Relief reported total revenue of $2,398,998,642 — of which 99.6% was contributions — total expenses of $2,307,002,152, and net assets of $1,248,271,923. Direct Relief is a 501(c)(3) organization (EIN 95-1831116); its tax exemption was issued in 1949.

Governance

Direct Relief follows a policy of full transparency: it routinely reports on how donations are used and makes its financial documents (tax returns, annual reports, audits), fundraising practices, and compensation policies open to the public, alongside published conflict-of-interest, whistleblower, and drug-regulatory-compliance policies.
